Mifflin Juniata Area Agency On Aging is Hiring a Care Manager Near Yeagertown, PA

Mifflin-Juniata Area Agency on Aging (MJ-AAA) is hiring a full-time Care Manager. Care Management is provided to consumers who require community based services to support their highest level of functioning while utilizing the most appropriate services. The care manager utilizes appropriate case work skill to assess the consumer’s needs, develop a person centered plan of care, arranges for services, and provides ongoing monitoring and support. Additionally, care managers are required to perform protective service activities including conducting investigations and crisis intervention.

Specific job responsibilities include, but are not limited to the following:

  • Receive referrals of individuals in need of aging services in specific geographic areas, and initiate contact within the required time frames.
  • Be familiar with local state and federal services and programs to benefit older persons and their families, and assist in accessing these programs, as appropriate.
  • Conduct care management activities including: assess (and reassess) consumers to determine unmet needs and available supports; develop care plans; arrange for services; follow-up and monitor as required; and other related activities.
  • Perform domiciliary care activities including: home evaluations; matching consumers and providers; monitoring placements; and, other related activities.
  • Complete all required protective service activities including: on-call and intake (phones), as assigned; complete (or receive) a report of need; conducting a thorough investigation; working with consumers, families, law enforcement and other agencies/supports and the court system to reduce risk and assure safety; petition the court and present testimony as needed; and other activities necessary to ensure the health and safety of PA consumers.
  • Complete all required forms, reports and documents in a timely, thorough, concise and legible manner.
  • Develop and maintain a strong working relationship with community social service agencies, law enforcement, and others that may be potential sources of help and assistance in care planning.
  • Develop and maintain a good working relationship with other AAA employees so that AAA services can be appropriately utilized and coordinated.

A Bachelor's Degree is preferred, but an applicant with an Associates Degree and relevant experience will be considered. The right candidate will have a passion for working with seniors and their families. Good communication and organization skills; and the ability to work both independently and as a team is a must. Full-time positions include a generous benefit package. This is a union eligible position.
