Mifflin-Juniata Area Agency on Aging (MJ-AAA) is hiring a full-time Care Manager. Care Management is provided to consumers who require community based services to support their highest level of functioning while utilizing the most appropriate services. The care manager utilizes appropriate case work skill to assess the consumer’s needs, develop a person centered plan of care, arranges for services, and provides ongoing monitoring and support. Additionally, care managers are required to perform protective service activities including conducting investigations and crisis intervention.

Specific job responsibilities include, but are not limited to the following:
A Bachelor's Degree is preferred, but an applicant with an Associates Degree and relevant experience will be considered. The right candidate will have a passion for working with seniors and their families. Good communication and organization skills; and the ability to work both independently and as a team is a must. Full-time positions include a generous benefit package. This is a union eligible position.
